The final evaluation report provides valuable learning experiences, enabling us to improve our grantmaking and to help nonprofit organizations in all communities served. Final Evaluation Report (PDF for reference only)
A final evaluation is due 60 days after 100% of grant-related activities are complete. Please contact the Foundation if there are any questions or an extension is required. If the grantee has a scheduled payment as part of a multi-year grant award, the payment will be held until the year-end evaluation report has been received and approved.
